Thank you for your participation in the 2012-13 Book Jackets program powered by AEP. Please complete our online submission form that can be found in the tabs above by grade level. It is important that you submit a separate form for each classroom. The maximum number of students on each form is 30. If you have more than 30 students, please complete an additional form.

You will receive a confirmation e-mail as soon as you submit your form. If you do not receive a confirmation e-mail, please call 614-246-3898 or email ccheek@bluejackets.com to confirm we received your submission. Shipping and handling of prizes will take 3-4 weeks.
